Pillar Home Health uses a collaborative approach, starting with understanding your needs, followed by a personalized consultation to develop a care plan, and finally, initiation of services.
Step one: Initial Phone Call
You will have an initial phone call with a member of the Pillar Home Health team.
This team member will listen to your needs and discuss the available care options with you.
Step two: In-Home Consultation
Pillar Home Health will then schedule a free in-home consultation.
The consultant team member will meet with you and your family to create a care plan tailored to your specific needs.
Our consultant will also explain the process of receiving care from Pillar Home Health during this time.
Step three: Select Start Date
Once the care plan is established, you will choose a start date to begin receiving care. We look forward to being your pillar.
